Receiver must conform received information by repeating orally and getting acknowledged for correctness

patient name and another unique identification
patient name and another unique identification on samples
Clinician identification if requested by clinician
Can be Department and Unit details where no single clinician can not be specified
Contact information
patient/clinician address
Can be Ward/OPD name/number where patient is located/visiting in future
Examinations requested
Note: Examinations to be performed in a given sample may be automatically selected during any stage of its processing, based on computerized algorithm in LIS(7.4.1.5.a)
Request forms must have additional remark fields for entry of details helpful to aid the laboratory in the selection of the examination methods, and the interpretation of the examination results (4.3.a)
Review requests received/accepted periodically and record findings. (6.7.1)
