While some details describe in scope are useful only to the laboratory personnel, others are useful to clinicians and patients.

Five most essential items on request form: MRD + Name + Department + Unit + Ward
MRD number is most important item in filling request form.
Example: SUR/14/00001234
Note that MRD number start with “SUR/YY/” whre YY is current year
The number is always of eight digits. Not writting complate 8 digits make it very difficult for the laboratory to retrive your report at a letter date.
Do not forget to write correct name. It is very useful to identify patient when any thing goes wrong with MRD number provided.
[Department + Unit + Ward ] help to locate patient for conveying critical reports. Please mention them without fail.
This is a no-fee, nominal-fee laboratory.Please request only nessesary examinations.
Special properites of sample, ie Fasting, post-prendial, pre-hemodyalisis needs to be indicated.
Specify type of non-blood sample as urine, CSF, peritoneal,plural,drainge location. This help laboratory select appropriate examination method for same analytes. e.g protein

The laboratory’s criteria for accepting and rejecting samples
All samples are precious. The laboratory donot reject them as far as possible.
Samples collected in unacceptable containers are rejected. (e.g Urea from fluoride tube).
Samples with request for no examination under scope are rejected. But samples with request where some of the requested examinations are under scope; are not rejected. Appropriate remark is made in the final report.
For incomplate details on request or sample containers,they are not rejected; but the transporter is explained to get complate details from the requesting clinicians.
Availability of clinical advice on ordering of examinations and on interpretation of examination results
Various guidlines and references listed below can be useful to clinicians while ordering examinations and their interpretations.

CSF rhinorrhoea
CSF have glucose in detectable amount, Nasal Secretion have very little.
CSF Glucose can mislead diagnosis in patients with DM

The laboratory gives patient sample report to
Authorized person from ward.
To authorized person of OPD sample collection centre (which also distibutes OPD reports).
Patient and their relatives.
Reports of samples received at special camps are delivered to authorized person managing the camps.
The laboratory use laboratory data for research only after striping data from patient identification.
Deviations and exclusions from, or additions to, the documented collection procedure
If clinician have deviated from procedure mentioned in this document, please indicate it in the request form to take informed decision by the laboratory and to include such deviation in the examination report
